Course Information

Studies have shown that patient wishes about care are often not known. Advance Health Care Directives may not be accessible, clear or followed by health professionals. POLST (Physician Orders for Life-Sustaining Treatment) helps clarify patient wishes and ensure that they are known and honored.

Purpose:
This workshop introduces health professionals to the new California POLST document and how it fits in the continuum of advance care planning.

In addition to learning about POLST, attendees will participate in interactive exercises to enhance their skills in facilitating meaningful conversations with patients and their families about POLST and goals of care.

Who Should Attend?
Physicians, nurses, case managers, social workers, chaplains and other interested staff from skilled nursing and acute care facilities, emergency medical services, home health and hospice.

Objectives:

  • Describe the target population for POLST completion.
  • Be able to facilitate a POLST conversation.
  • Be able to complete a POLST document.
  • List two signatures required on the POLST.
  • Describe the differences between an Advance Health Care Directive (AHCD) and the POLST document.
  • Discuss the importance of palliative care with patients and family members.
